The mission of the Florida Outreach Center for the Blind is to develop and administer programs that will integrate blind and visually-impaired persons into the social, economic, and spiritual lives of their community.
Florida Outreach Center for the Blind, Inc. believes that individuals who are blind or visually impaired have the right to the tools and training that will allow them to reach their personal, academic, and vocational goals and to live their lives independently.

            Alzheimer's Community Care                         www.alzcare.org 

ACC is the largest provider of community-based, dementia-specific services in Florida. The grant will be used to fund the Art for Alzheimers Program at the Lake Worth Day Center. Alzheimer’s Community Care is dedicated to promoting and providing community-based, family-centered care for patients and their caregivers living with neurocognitive disorders, through the belief, that where there is help, there is hope. 

Fresh RX – Patient Produce Program   www.freshrx.org/patient-program

FreshRx Farm to Hospital is a fresh produce prescription program working with oncology, cardiology, neurology (stroke), and pediatric patients in the community whose doctors have advised them to change their diets as part of a comprehensive treatment plan. FreshRx is committed to buying produce from local family farms that use organic growing practices; that means all the produce is grown without the use of synthetic pesticides or fertilizers. Participants receive a free 16 week farm share which allows them to start incorporating more fresh produce into their diet without adding any additional financial pressure.
This program is intended to serve patients that are nutritionally vulnerable, including those on Medicare, Medicaid or paying out of pocket for medical expenses.

Elder Crisis Outreach is a service devoted solely to serving elders, 60 years of age or older, their families and/or caregivers by providing Supportive crisis counseling, Comprehensive needs assessment,Goal setting and a plan of action, Linkage to community services, Advocacy with family or service providers, Suicide evaluation and much more. 211's Elder Crisis Outreach is available to qualifying elders over the age of 60, individuals who are housebound or disabled and are residing in Palm Beach County

          Boynton Beach Faith Based CDC   www.boyntonbeachcdc.org

The mission of the Boynton Beach CDC is to  develop sustainable communities through services and resources that include home ownership opportunities, real estate development, financial education, and youth empowerment, resulting in the spiritual and economic transformation of people. They do that through great programs like these for seniors.

AGING IN PLACE Senior Care: Advocacy services to assist seniors to remain in their homes rather than enter institutional care by providing basic social services that address health concerns associated with senior isolationism, on-line shopping and FREE delivery, virtual social activities, LIHEAP and EHEAP utility payments, internet access and drive-by birthday celebrations.

CHAMPION SENIOR Mobile Library: Provides access to resources for mobility aids such as walkers, crutches, wheelchairs and life alert services are available to assist seniors in maintaining their independence at home and in their respective communities.

Clinics Can Help makes your donations of medical equipment available to others who need it. Working together, we build hope, provide dignity and save lives throughout South Florida. The grant will be used to fund the purchase of supplies needed for the lending closet.

          Hands Together for Haitians Health and Wellness Initiative             www.handstogetherforhaitians.org/

Their mission in Palm Beach County (Florida) is to assist the Haitian immigrant population with transition from living in their native country to living in the United States. They hold weekly presentations to the elderly Haitian community on topics such as chronic health concerns and hospitalizations, health insurance and benefits, including Medicare, medication plans and more.

The Palm Beach County Partnership for Aging (PFA) makes available a limited number of one-time grants to community organizations that enhance the lives of seniors in Palm Beach County.

Eligibility:

1. Grants are limited to registered non-profit and governmental organizations serving Palm Beach County seniors ages 65 and older.

2. 100% of the grant funds must directly impact seniors. Examples include: temporary cash assistance or to assist with the purchase of services or supplies.

3. It is expected that grants will be used within one year of receipt. Grantees are asked to report their outcomes to PFA after one year has transpired.

4. No part of the award may be used to cover administrative costs – such as employees’ salaries -- or other expenses unrelated to direct services to seniors.
